نقش کاردرمانی در سکته مغزی

سکته مغزی یک‌فوریت پزشکی است که درصورت عدم‌تشخیص و درمان فوری می‌تواند باعث آسیب دائمی عصبی، ایجاد عوارض و مرگ شود.

سکته مغزی یکی از اختلالات عملکردی پیچیده است که با آسیب به مغز به‌وجود می‌آید و منجر به اختلال در نورون محرکه فوقانی می‌شود و سبب فلج یا ضعف در یک طرف بدن(شامل: تنه، اندام‌ها و گاهی اوقات نیز صورت و ساختارهای دهانی سمت مقابل نیمه آسیب دیده مغز)می‌شود. علاوه بر فلج حرکتی، یکسری از اختلالات عملکردی ازجمله اختلالات حسی، اختلالات درکی، نقص‌های بینایی، تغییرات شخصیتی و عقلانی و مجموعه پیچیده‌ای از اختلالات گفتاری و زبانی را نیز به همراه دارد.
تقریبا 50 درصد کسانی که از سکته مغزی جان به‌درمی‌برند دچار مشکلات و اختلالات حرکتی دائمی در زمینه حفظ تعادل در حالات و وضعیت‌های مختلف و عدم‌توانایی جهت ایستادن و راه رفتن به تنهایی و بدون‌کمک گرفتن از دیگران و همچنین عدم‌توانایی جهت انجام فعالیت‌های روزمره زندگی از قبیل لباس پوشیدن، غذا خوردن، حمام رفتن و… هستند که برای غلبه بر این مشکلات و نقایص نیازمند برنامه‌های توانبخشی ازجمله کار درمانی درهمان روزهای ابتدای سکته هستند؛ به‌طوری که، درمان‌های کاردرمانی این عدم‌استقلال و وابستگی را به حداقل می‌رساند. با این حال، حدود و میزانی که کاردرمانی، پیامدهای پس از سکته مغزی را بهبود می‌بخشد برای بیماران مختلف متفاوت است.
یکی از اصول کاردرمانی بیماران سکته مغزی، انجام تمرینات کششی و تقویتی روی مفاصل و تاندون عضلات برای حفظ دامنه حرکتی مفاصل و کم‌کردن سفتی عضلات است که با این‌کار می‌توان میزان معلولیت بیماران را کم و به استقلال آنان کمک کرد.
یکی دیگر از اصول مهم در کاردرمانی و اصولاً توانبخشی این بیماران تأکید بر استفاده از سمت مبتلاست. بنابراین از همان ابتدا باید به بیمار تأکید کرد که سعی کند از دست و پای سمت مبتلای خود استفاده کند، حتی اگر این کار با سختی و دشواری و به‌صورت ناشیانه انجام شود، که برای این کار، می‌توان مثلاً سمت سالم بیمار را مهار کرد و او را مجبور کرد که از سمت مبتلا استفاده کند. البته برای شروع این کار باید با استفاده از یکسری تکنیک‌های خاص کاردرمانی، سمت مبتلا را برای حرکت کردن آماده کرد. انجام این کار باید توسط یک کاردرمان صورت پذیرد و بعد از اندکی بهبود می‌توان این کار را به خانواده بیمار آموزش داد.
سعی شود تمرینات کاردرمانی مبتنی بر وضعیت‌های عملکردی و همسو با فعالیت‌های روزمره زندگی بیمار باشد؛ یعنی اینکه مثلا اگر هدف ما دادن حالت‌ها و وضعیت‌های مختلف به بیمار است باید از سطوح پایین‌تر که نشستن است شروع کنیم و بعد سراغ سطوح بالاتر یعنی دو زانو ایستادن برویم. وضعیت‌های عملکردهای روزمره شرح داده شده در بالا عبارتند از: حفظ تعادل در وضعیت نشسته، درازکردن دست و گرفتن اشیا، برخاستن از روی صندلی، حفظ تعادل در وضعیت ایستاده و بالاخره راه‌رفتن. این عملکردهای پایه را باید در مورد هر بیمار ارزیابی کرده و تمرینات عملکردی مناسب را با توجه به وضعیت بیمار، طراحی و اجرا کرد. در مراحل اولیه ممکن است استفاده از برخی وسایل کمکی مثل واکر به‌ویژه در مورد راه‌رفتن، ضرورت پیدا کند.
اصل مهم در انجام این تمرینات تکرار زیاد است چون با تکرار زیاد است که مغز می‌تواند تا حدودی عملکرد منطقه آسیب‌دیده ناشی از سکته را جبران کند. مطلب دیگر در کاردرمانی بیماران سکته مغزی، مشارکت اطرافیان در توانبخشی بیمار است. کاردرمان باید نحوه انجام تمرینات را به اطرافیان بیمار آموزش دهد تا با این کار آنها را در درمان بیمار سهیم کند، زیرا تمرینات کاردرمانی و توانبخشی زمانی موفق است که با شدت بالا و تکرار زیاد انجام شود و در این صورت است که بهبود حاصل می‌شود. بر این اساس، نباید تمرینات کاردرمانی فقط مختص به زمان بستری بودن بیمار در بیمارستان باشد بلکه تمرینات باید در منزل نیز توسط کاردرمان و اطرافیان ادامه یابد.
